Causes and Treatment of Hypertension in Patients with PKD

PKD is a hereditary disease and has many symptoms and complications. For example, hypertension and renal ischemia. In this way, what are causes of hypertension and renal ischemia?


Hypertension is common and occurs in a majority of autosomal dominant polycystic kidney disease (ADPKD) patients before the loss of kidney function. The kidneys play a vital role in controlling blood pressure. Having ADPKD greatly increases the chance that high blood pressure will develop. About half of people with ADPKD aged 20-34 years have high blood pressure, and most people with ADPKD will develop high blood pressure at some point in their life. High blood pressure in itself does not usually cause symptoms. So, it is easy to be unaware that you have high blood pressure.


Hypertension relates to progressive kidney enlargement and is a significant independent risk factor for progression to ESRD. PKD1 and PKD2 expression levels are highest in the major vessels and are present in the cilia of endothelial cells and in vascular smooth muscle cells. Decreased or absent polycystin 1 or 2 expression is associated with abnormal vascular structure and function. Pkd1/Pkd2 deficiency results in reduced nitric oxide (NO) levels, altered endothelial response to shear stress with attenuation in vascular relaxation. Ten percent to 20% of ADPKD children show hypertension and the majority of adults are hypertensive before any loss of kidney function.


However, untreated high blood pressure can cause further damage to the kidneys, and greatly increases the risk of developing heart disease and stroke. Therefore, early detection and treatment of high blood pressure are important. Traditional Chinese Medicine makes a significant effect on PKD treatment.
