Hypercoagulable State Is The Obvious Symptom Of Patients With Nephrotic Syndrome

The most direct evidence of hypercoagulability in nephrotic syndrome is an increase in plasma fibrinogen levels. The rate of plasma fibrinogen catabolism in patients with nephrotic syndrome is normal, and the increase of plasma fibrinogen level is due to the increase of its synthesis.

In addition, fibrin and fibrinogen degradation products also increased significantly. Plasma fibrinogen and metabolic disorder can disappear with the recovery of nephrotic syndrome. Coagulation factors Ⅴ, Ⅶ, Ⅷ and Ⅹ also increased, which may be related to the increase of liver synthesis. Increased platelet aggregation is also one of the reasons for hypercoagulability in nephrotic syndrome, which may be related to hypoproteinemia, hyperlipidemia and the decrease of sialoglycoprotein content in platelet membrane to prevent platelet aggregation. Fibrinolytic and anticoagulant system abnormalities include a decrease in plasminogen level ( although the level of plasminogen activator increases ), an increase in A2 – macroglobulin, and a decrease in antithrombin III ( lost from urine ). Anti – plasmin, anti – trypsin A1 plasminogen activator and endothelial prostacyclin stimulator were also reduced.
